Original Entry register cards are white; Promotional register cards are yellow.
Register cards for examinations administered as a result of a Request for Prior
Approval of Compensatory Qualification(s) for Examination have one stripe; register
cards administered as a result of a Request for Prior Approval of Waiver of Qualification(s)
for Examination have two stripes.
The Employer completes ONE register card and Notice of Examination Score for
each examination given.
The original register card is placed on the appropriate register at the place
of employment for which the applicant applied for employment.
All register cards must be signed by the applicant and by the DER. A DER may
delegate the right of signature to a member of his/her staff, in which case
the initials of the individual signing for the DER must be shown. The DER may
use a signature stamp, in which case the initials of the individual using such
stamp must be shown.
Each place of employment shall retain all register cards/information for at
least five years after the date of placement on the inactive file.
